Job summary

The Integrated Case Management Team is seeking an experienced and motivated Band 6 Senior Occupational Therapist to join our multidisciplinary service delivering hospital-level care in patients' homes. Working closely with community and acute partners, the service supports patients referred from GPs, Rapid Response, A&E, and other services, carrying out holistic home-based assessments and developing person-centred treatment plans in collaboration with consultants through virtual ward rounds.

You will work within a supportive multidisciplinary team, including Consultants, GPs, Nurses, Clinical Pharmacists, Physiotherapists, and Occupational Therapists, supporting patients in the community for up to six weeks to stabilise health, promote independence, and prevent avoidable hospital admissions. We are looking for a senior clinician with strong clinical judgement, excellent communication skills, and a passion for delivering high-quality, patient-centred care across traditional boundaries.

Main duties of the job

As a Band 6 Occupational Therapist, you will:

  • Independently undertake initial and follow-up patient visits, with joint visits arranged where clinically appropriate.

  • Develop a holistic, person-centred approach to assessment and treatment, building advanced clinical skills across a range of presentations.

  • Undertake extended scope activities, including venepuncture, advanced assessment of acutely unwell patients, and acute injury assessment, in line with training and competency.

  • Gain broad clinical experience across generic, orthopaedic, musculoskeletal, respiratory, neurological rehabilitation, and falls prevention.

  • Contribute to the day-to-day operational management of the team, including participating in handovers and supporting treatment planning through virtual ward rounds.

  • Provide supervision and support to junior staff and students.

  • Engage in in-service training and ongoing Continuing Professional Development opportunities.

  • Registration with the Health and Care Professions Council
  • Evidence of continuous professional and personal development relevant to rotations
  • Wide ranging experience including musculoskeletal outpatients, adult/older people's rehabilitation, community rehabilitation and neurology placements
  • Registration with the Royal College of Occupational Therapy
  • Evidence of leadership courses/training
  • Training on effective appraisal

Desirable

  • Registration with a special interest group relevant to the rotations

  • Able to manage a caseload efficiently
  • Demonstrate professional independence, while recognizing boundaries of own practice
  • Effective occupational therapy skills to be able to assess, goal set, treat and manage rehabilitation patients particularly within a multi-disciplinary team.
  • Problem solving approach to clinical interventions
  • Able to keep logical and accurate patient records
  • Able to present information, written and orally, in a clear and logical fashion
  • Community experience
  • Relevant post-qualification experience in acute clinical area(s)
