Job summary
We are seeking a skilled and motivated Occupational Therapist to provide specialist neurological assessment and rehabilitation for people with complex neurological conditions, primarily within their own homes and community settings across Kensington & Chelsea.
This role sits within a community-based multidisciplinary neurorehabilitation team, delivering high-quality, person-centred care across a range of pathways. These include the Early Supported Discharge Service for Stroke, the Level 2 Specialist Neuro Rehabilitation Outreach Service (delivered in partnership with Imperial), and the wider Community Neuro Rehabilitation Service for people with acquired or progressive neurological conditions.
The post holder will work as an autonomous practitioner, managing a defined caseload while being actively supported within a collaborative and inclusive team environment. The role encourages professional accountability, reflective practice and shared learning, recognising that high-quality outcomes are achieved through openness, trust and continuous improvement.

Main duties of the job
The post holder will:
- Provide specialist occupational therapy assessment and intervention for people with complex neurological conditions using a person-centred, evidence-based approach
- Undertake multidisciplinary neurological assessments, working with service users, carers and the wider MDT to identify needs and shared goals
- Complete specialist assessments including functional, cognitive, seating and upper limb assessments to inform intervention.
- Develop and deliver individualised treatment programmes using clinical reasoning and a range of therapeutic approaches
- Take responsibility for clinical decisions and care plans, reviewing outcomes and adapting interventions as required.
- Formulate person-centred goals and clearly reasoned prognoses in partnership with service users and carers
- Monitor progress using recognised outcome measures and adjust treatment plans in response to change
- Work collaboratively with multidisciplinary colleagues to deliver a coordinated service
- Communicate clearly and compassionately with service users, carers and professionals
- Contribute to an inclusive team culture through shared learning and respectful communication
- Maintain accurate and timely documentation in line with clinical and governance standards

- Current AHP with UK HCPC registration
- Experience of working with neurological patients in delivery of rehabilitation packages or in a rehab setting at Band 5 level.
- Documented evidence of professional development and reflective practice in relevant aspects of neurological rehabilitation e.g. attendance at in-service training sessions, reflective practice
Desirable
- Attendance at relevant post-graduate neurological training courses e.g. spasticity management , splinting, cognition and perceptual assessment , fatigue management
- Experience of working in a neurological rehabilitation setting at band 6 level.
