Job summary
Athlone Rehabilitation Unit is a 22 bedded in-patient rehab unit. We provide multidisciplinary rehabilitation for patients from age 18 and above who require a period of inpatient physical rehabilitation. We have a sister site called Alexandra Rehab Unit which has 11 beds and cross cover may be required.
We are seeking a motivated, caring, and dynamic Rehabilitation Nurse with experience in stroke, to apply for this exciting opportunity to work in our team. The Nurse will work closely with the Multidisciplinary Team consisting of Doctors, Matron, Ward Manager, Staff Nurses, Health Care Assistants, Physiotherapists, Dieticians, Speech and Language Therapists, Podiatrists, Pharmacists, and Domestic Staff.
The Nurse will assist the Ward Manager to provide nursing care to, and support recovery of, patients to meet their rehabilitation goals, focusing on the needs of the patients rather than the illness or condition. Also work closely with other Allied Health Professionals and Social Care services, to maximise patient's recovery, building confidence to regain their control and improve their quality of life.
Main duties of the job
- Work as part of a multidisciplinary team, providing nursing care and rehabilitation to inpatients in the rehabilitation unit.
- Undertake comprehensive nursing assessments without supervision, determining patient needs, risks, and planning, implementing, and evaluating care accordingly.
- Act as the named nurse for designated patients, ensuring continuity of care and coordinating discharge planning with other services.
- Manage admissions, provide care and discharge planning while ensuring effective service delivery, in the absence of senior colleagues, take charge of the rehabilitation unit,
- Contribute to maintaining high standards of clinical nursing care using an evidence-based approach, in line with clinical governance principles.
