We're seeking a highly motivated & experienced clinician to join our End of Life Care Team in Merton as aCommunity End of Life Care Nurse. This is a unique opportunity to shape quality palliative care across our community.

Pivotal Role in Quality: This isn't just clinical delivery, it's about facilitation, mentorship, and policy implementation. You'll be an essential link, working collaboratively to embed best practice & Coordinated Care across primary care, social services & care homes.

Expert Autonomy: While supported by the Band 7 Specialist Nurse, you'll work with a moderate degree of autonomy, utilising your clinical judgement & discretion to provide direct advice & mentorship to front-line staff caring for dying patients.

Strategic & Clinical Development: This post offers an immediate chance to develop your clinical leadership & strategic skills. You'll be instrumental in managing a patient caseload to facilitate best practice & collecting data to support key performance indicators (KPIs) for End of Life Care.

Collaborative Environment: You'll be a key partner across the Merton community, working closely with GPs, other specialist nurses, case management teams & the voluntary sector to ensure service sustainability.

- Registered Nurse with full and current UK NMC registration
- Evidence of continuing personal development and training
Desirable
- Related Diploma/Module in EoLC, Palliative Care, Care of the Dying
Experience
Essential
- Post-registration experience
- Previous experience of palliative care / Previous experience of the End of Life Care tools (Gold Standards Framework and Advance Care Planning, Coordinate My Care)
- Experience of working in the community
- Familiar with EoLC medications and how to administer via subcutaneous route
- Demonstrate a knowledge of national policies influencing end of life care
- Experience, insight and understanding of Clinical Governance, risk management, clinical effectiveness, audit, patient and public involvement initiatives
- Competent using the Universal Care Plan
Desirable
- Teaching experience in formal and informal settings
- Recent experience of involvement in management of patients moving between acute care and community
