CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

Following significant investment into community stroke services for Herts Valleys, we are now welcoming applications from experienced Rehabilitation Assistant with an interest and, ideally, experience in stroke rehabilitation to work collaboratively within the new Integrated Community Stroke Service (ICSS).

This modern stroke service infrastructure follows the national service model and is delivered by a multidisciplinary team offering stroke rehabilitation, in patients own homes in and around West Hertfordshire, at a range of intensities.

Within the model we have the following:- Early Supported Discharge (ESD), commonly a course of high-intensity, home-based rehabilitation that a patient receives over up to 6 weeks following a discharge for mild-moderate new stroke.- Neurological Enhanced Transition Team (NETT), commonly a course of home-based high-intensive rehabilitation delivered up to 6 weeks following a discharge for moderate to severe new stroke or other neurological conditions.- Community Stroke Team, which is for patients experiencing a change in their daily function directly related to a pre-existing stroke injury.

Main duties of the job

This is a great opportunity for someone who is conscientious and adaptable. The successful candidate will have excellent communication skills and will be able to demonstrate an adaptable approach whilst meeting guidelines and stroke standards as well as being part of the new stroke pathway. They must be confident working as a lone worker, with the support of the team.

You will be working with people in their own homes, helping them to achieve their rehab goals, supporting therapy programmes set by Speech and Language Therapists, Physiotherapists, Occupational Therapist and Psychologists.

You will have the opportunity to be involved in developing provision for stroke patients, working in a supportive, friendly and passionate multidisciplinary team. You will form an essential part of a dedicated, knowledgeable multi-disciplinary team that regularly engages with team and individual clinical supervision. Development and training opportunities are available through the CLCH Academy.

This role involves working with patients in their own home, covering West Hertfordshire.

  • Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3)
  • OR A recognised equivalent course in health and/or social care validated by the Regulated Qualification Framework (RQF) or a Level 3 or higher course in a health and/or social care subject from a Higher Education Institute
  • OR A formal academic qualification in any subject from a college or university of Level 3 or above PLUS two years care delivery in a health or social care setting

Desirable

  • GCSE grade 9 - 4 or equivalent in English and Maths
  • Registration with the national regulatory body (when it becomes available).
  • Willingness to undertake further training as appropriate

Experience

  • Experience of working as part of a team
  • Working experience or knowledge of the NHS

  • Previous experience as a healthcare or therapy assistant
