Band 7 Highly Specialist Occupational Therapist, Discharge to Assess and intermediate care team, Central London Community Healthcare NHS Trust (CLCH) - Wandsworth. Fulltime

Are you a Band 7 or an experienced Band 6 Occupational Therapist looking for an exciting, new challenge and a chance to take on greater responsibilities?

We are looking for a highly specialist, dynamic and motivated Occupational Therapist with excellent clinical skills and a positive attitude and commitment to providing excellent holistic community-based care. The person will join our group of AHP staff in a permanent role and supporting an MDT community therapy service.

Discharge to assess (D2A) and the intermediate care team is a 7 day a week community therapy/care assessment and rehabilitation team in Wandsworth. It provides therapy services to specific types of patients upon discharge from hospital and in the community. These patients are often more urgent and/or complex. The team supports the patient to ensure readmission is avoided. This involves risk management, disability management and rehabilitation.

Main duties of the job

The team sees a wide variety of conditions/pathologies and provides therapy in a holistic way using a wide variety of knowledge and skills. The team consists of occupational therapists, physiotherapists, rehabilitation assistants and health care assistants in a multidisciplinary team.Main duties will include;

  • Experience of working within an intermediate care team
  • Work as a highly skilled senior practitioner with expert knowledge and work alongside clinical lead to develop the service
  • Providing highly specialist occupational therapy care to a community based caseload.
  • Carrying out robust triage of patients referred to the team to ensure their safety and optimal pathway allocation
  • Ability to provide leadership within the occupational therapy and wider team.
  • Experience of line management and supervising others. Supporting and deputising for the Clinical Lead and Operational manager of the service
  • Opportunities to take the lead on service improvement opportunities, develop your managerial skills and suggest and lead on audit and research projects and other teams in the intermediate care team

Essential

  • Degree or diploma in occupational therapy
  • Registered with the Health Care Professions Council
  • Up to date relevant knowledge through CPD as evidenced by portfolio
  • Hold relevant right to work in the UK documentation

Desirable

  • Membership of Royal College of Occupational Therapy and / or relevant special interest groups
  • Attendance at Clinical Educator Course
  • Leadership and management training

Experience

Essential

  • oPrevious NHS experience
  • oA clear understanding of current NHS issues, professional standards, CPD, clinical governance and experience of audit
  • oAppropriate post-qualifying experience in a range of areas acute care of the elderly, neurology and community
  • oAppreciation of constraints to working in the community
  • oAbility to assess patient conditions, establish the patients problem, formulate and implement and treatment plan

  • Experience of MDT working and understand their roles
