Main duties of the job

  • Integrated Team Collaboration:Work closely with the multidisciplinary team to deliver holistic care, ensuring seamless service delivery for patients and communities.
  • Patient-Centered Care:Partner with patients and their families to create personalised health plans that address their unique needs and goals, ensuring their voice is central to the care process.
  • Service Development:Actively engage in service improvement initiatives, helping to shape and enhance the care we provide to meet the evolving needs of our patients.
  • Leadership and Deputising:Support the Team Leader in the smooth running of the service and step into the Team Lead role when necessary, ensuring continuity of care and leadership.
  • Collaboration with Providers:Work alongside other healthcare providers to ensure the efficient and effective delivery of services, fostering collaborative relationships across health and social care.

  • A registered nurse with experience in community healthcare and a strong understanding of district nursing.
  • A compassionate, proactive individual with excellent communication skills, able to build meaningful relationships with patients, families, and colleagues.
