We're looking for a motivated, forward-thinking Community Diabetes Specialist Lead Nurse to join our friendly community diabetes team in Brent and Harrow.

You'll lead and support nurses, allied health professionals, and lay educators to deliver safe, effective, and compassionate diabetes care closer to home. The role combines leadership and clinical practice, involving oversight of day-to-day service operations, supervision, and quality improvement.

Main duties of the job

This is a broad summary of the role detailing key areas of operation and responsibilities - further detail will be provided in the Main Duties and Responsibilities section, below, which provides more granular detail.

  • To be responsible for the operational management and day to day running of the community diabetes service within an identified locality within the ICB.
  • To work jointly with the diabetes leadership team across Brent and Harrow in ensuring the service meets trust and commissioner set key performance indicators and standards.
  • The post holder is expected be an experienced diabetes specialist nurse, aware of the full scope of clinical practice and expected to support and supervise colleagues and team members.
  • To be a key member of the multidisciplinary Community Diabetes Service to plan and deliver high quality integrated diabetes care in the community.
  • To ensure efficient and cost-effective processes and ways of working are in place.
  • To oversee the line management and coordination of supervision and rotations to ensure these are being undertaken appropriately.
  • You will be expected to carry at least a 60% clinical caseload and will be supported with a job plan.

Essential

  • Degree in Nursing or equivalent.
  • Current registration with the NMC, Showing a registered General Nurse.
  • Relevant post registration training in Diabetes eg: Masters or diploma in diabetes management.
  • Independent Nurse Prescriber (or willing to undertake)

Desirable

  • Mentorship training.
  • Supervisory skills training
  • Post registration training in Cognitive Behavioural Therapy.
  • Management qualification

Experience

Essential

  • Minimum of at least 3 years' experience in managing complex diabetes cases.
  • Experience in managing a clinical case load.
  • Experience in the training of students
  • Experience in organising and implementing training/teaching. programmes for healthcare professionals and clients.
  • Excellent group education and presentation skills
  • Experience of conducting audits and/or research.
  • Experience in contributing to Service Development, including policy and guideline development
  • Experience in supervising others.
  • Experience working in a multi-disciplinary team
