Job summary
The Barnet Children's Continuing Care Team is a small, friendly, dedicated team of nurses who oversee and case manage the processes of delivering Continuing Health Care required for children and young people in Barnet with complex health needs. The children and young people eligible for Continuing Health Care are identified in accordance with the National Framework for Children and Young People's Continuing Care 2016.
Our children, young people and their families are at the heart of everything we do. The post holder will work within the team and with partner agencies across Barnet to ensure that commissioned packages of care is of high quality, safely delivered and based on individualised needs.
The post holder will be part of the Barnet Children's Continuing Care Team and, in a role, ideally positioned to make a positive difference to the lives of children and young people with complex health needs. The team is committed to developing its staff and opportunities are available to enhance your professional development.
Main duties of the job
- The post holder will undertake Children's Continuing Care assessments and reviews for theeligibility of continuing care packages. These children may have life-limiting / threateningconditions and/or may be technology dependent.
- The post holder will be a designated resource to children and their families within the caseload, therefore requiring liaison with multidisciplinary teams, other community services, andworking across organisational boundaries within the London Borough of Barnet.
- The post holder will be responsible for ongoing case management of care packages for thechild and families until they transition to adult services, or they are no longer eligible forChildren's Continuing Care
