An exciting opportunity has arisen for a Band 3 Rehabilitation Support Worker to join our Barnet Integrated Community Service by providing a high quality service to our housebound patients by facilitating earlier hospital discharges and preventing unnecessary hospital admissions.
Main duties of the job
As a Band 3 Rehabilitation Support Worker you would be responsible for providing safe, personalised and effective care to patients who require community therapy care by working alongside the community nursing team to deliver clinical and administrative duties.
Your main duties will include providing a number of clinical activities and holistic care under the direct delegation of a registered therapist using a wide range of clinical skills and interventions within a home environment.
You will need to possess excellent Communication skills and be a team player. You will also need to have good IT skills to support the systems in place.

- NVQ Level III
Desirable

- Minimum of two years' experience of working in health or similar post/s.
- Experience of working in the Community setting
- Experience of working with elderly people and in particular frail elderly people and people with physical disabilities

- Knowledge of the role of a Rehabilitation Assistant in a community rehabilitation team

- Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
- Demonstrated acceptable level of numeracy and literacy skills, and proficient IT skills - especially Microsoft packages.
- Basic competency training in: Mobility assessment, individual exercise programme, falls prevention.
- Experience in assessment of Personal and Domestic Activities of Daily Living and carrying out these tasks with clients to improve ability

- Basic knowledge of common medical conditions and a basic understanding of their impact on functional ability
- Knowledge and experience of using outcome measures appropriate for fallers.
- Basic knowledge and understanding of the National Service Framework for Older People
