Are you a GP with a passion for rehabilitation and older adult care?We're looking for an experienced and enthusiastic GP Clinical Lead to join our well-established, supportive multidisciplinary teams at Athlone and Alexandra Rehabilitation Units in West London.
This is an exciting opportunity to play a key clinical leadership role in the North West London Integrated Care System, supporting patients who are stepping down from acute hospital settings and require specialist rehabilitation.
You will work as part of a team of doctors to provide day-to-day medical care for patients on our two rehabilitation units and lead on clinical governance, service development, and quality improvement.
Main duties of the job

Provide medical leadership and care for patients on the bedded rehabilitation units.

Carry out comprehensive assessments, care planning, and regular ward rounds.

Work closely with nursing, therapy, and medical colleagues to deliver coordinated rehabilitation care.

Provide clinical input into multidisciplinary team meetings and decision-making.

Ensure timely and accurate completion of clinical documentation, discharge summaries, and prescriptions.

Review deteriorating patients promptly and escalate as required.

Offer expert clinical advice and support to colleagues across the service.

Contribute to audit, research, and service improvement initiatives.

Support and mentor colleagues, contributing to training and professional development.
