Our Specialist Palliative Care services in Harrow are offering an exciting and innovative opportunity for a community nurse to join our team.

Main duties of the job
- You will work as part of the Specialist Palliative Care Team
- You will work in partnership with patients and their families to develop and agree tailored health plans to address their need
- You will work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities
- You will support learners

- Experience of undertaking general assessment by utilising evidence based care.
- Previous experience in the Nursing process, care planning and fluent in record keeping
- Experience of providing holistic care that involves the multidisciplinary team
