Job summary

Central London Community Healthcare NHS Trust (CLCH) is seeking an experienced and compassionate Consultant to join our specialist multidisciplinary team at Pembridge Community Specialist Palliative Care service. This is a unique opportunity to lead and shape the delivery of holistic palliative care in one of London's most diverse and vibrant communities.

CLCH is the largest community healthcare provider in London, delivering over 60 services to 600,000 people across Westminster, Kensington & Chelsea, and Hammersmith & Fulham. Pembridge , part of CLCH's Inner Division, Pembridge Community Specialist Palliative Care service provides high-quality NHS-funded palliative care through a large community team, a day hospice, and outpatient facilities.

You will provide clinical leadership across community and day services.

You will lead and provide specialist palliative care to patients in the community and take Consultant responsibility for patients across settings including home visits, day services and outpatient clinics.

You will mentor and supervise junior doctors and medical trainees and work closely with the senior nursing team and managers to ensure safe, effective, and compassionate care delivery.

You will also act as clinical lead for the service, contributing to strategic planning, service development, and clinical governance.

Main duties of the job

  • The post-holder will provide specialist palliative care to patients in the community, day services, and outpatient facilities.
  • The post-holder will have continuing responsibility for patients in his/her care and will undertake administrative duties associated with care of patients and the running of the clinical departments.
  • The clinical work will be based within the community with the Specialist Community Palliative Care Team, day therapy services and out-patient services.
  • Provide robust specialist palliative consultant care across care settings, providing diagnostic and treatment support to colleagues as well as advice and support to patients and families.
  • Provide clinical support to specialist palliative care nurses, other clinical nurse specialists, allied healthcare professionals and other team members, leading change in practice related to supportive and palliative care across the Trust.
  • Lead the multidisciplinary teams delivering specialist palliative care service to community patients, day therapy unit and outpatient attendees.
  • Be responsible for decisions and patients discussed at the weekly community MDT.
  • Be responsible for reviewing patients at home and in outpatients as required by the MDT and liaising with GP and others.
  • Conduct regular caseload reviews of the community CNS's.
  • Ensure active engagement and support of End of Life care in the community.

  • Full GMC registration.
  • Entry on GMC specialist register for Palliative Medicine or related specialty, CCT (due within 6 months of interview) or has a CESR
  • Membership of relevant Royal College or equivalent
  • Work permit for the UK.

Desirable

  • Postgraduate higher qualification: PhD, Diploma or Master's degree in an appropriate subject.
  • Accredited clinical/educational supervisor
  • Qualified medical appraiser
  • Advanced Communication Skills certificate / training.

Clinical Skills

Essential

  • Excellent clinical skills
  • Ability to supervise the clinical work of consultants, doctors in training and other staff
  • Extensive experience in palliative medicine
