CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!
An exciting opportunity has arisen for aBand 4 Rehabilitation Assistant to work within the Hertfordshire Inpatient rehabilitation units.
This particular post will involve supporting patients with exercise programmes and functional rehabilitation for patients on a Non Weight Bearing pathway within a care home setting, at Forest Care Village, Borehamwood.
We are looking for a motivated individual to join the team, who has the passion and skills for supporting the therapy team in the delivery of a high standard of care. You will be working with a multidisciplinary team that includes Physiotherapists, Occupational Therapists, Nurses, care home staff and the Social Care Team.
Main duties of the job
As this role is a band 4 position you will undertake non complex assessments of patients as directed by the therapists. The successful candidate will also be involved in multi-disciplinary team meetings to participate in discharge planning and co-ordinate the patients care through the pathway.
The successful candidate will have access to a comprehensive induction programme, ongoing in-service training to develop their skills within the role to achieve the required competencies and access to development opportunities provided by the CLCH Academy.
This post is part time 0.76 WTE (28.5 hours a week) Maternity cover. There is seven-day working within the rehabilitation units within Hertfordshire and the successful candidate may be required to cover weekend shifts in these units. The weekend rota is expected to be 1:6.

- Formal qualification in one of the following: Psychology/ Social Sciences / Biological sciences/ NVQ in Health and social care
- Member of relevant specialist interest group. NVQ Therapeutic and diagnostic support level
Experience

- Experience of working with people with physical disabilities
- Experience working as a member of a team
- Knowledge of recent NHS developments and how they may impact on patient care
