CLCH Hertfordshire Adult Community Services -CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

**Due to internal promotions, we have vacancies for Senior Nurses/Paramedics within the Hertfordshire UCR (Urgent Community Response) Service **

We are looking for forward thinking, passionate staff who can take a positive risk management approach to support people at risk of a hospital admission to remain at home when acutely unwell, within our UCR service West Hertfordshire- previously known as the Herts Unplanned Care Team.

The aim of the service is to avoid unnecessary admissions into secondary care and provide a timely discharge from hospital and community bedded units for patients requiring urgent therapy input in their homes. The team provides care across three pathways delivered over 7-days a week 8am to 10pm - Rapid Response (RR), Early Intervention Vehicle (EIV) and Discharge to Assess (DTA). All pathways are delivered in a patient's usual home environment.

Main duties of the job

We are looking for proactive and enthusiastic clinicians (Nurse or Paramedic) to join our innovative UCR service and enable the delivery of care to patients in their own homes across Herts Valleys.

Rapid Response (RR) focusses on preventing avoidable hospital admissions for patients with acute ill-health conditions by providing holistic assessment and treatments up to 5 days. This pathway also delivers face to face assessments and treatment for patients under virtual hospital pathways.

Discharge to Assess (DTA) pathways aim to support the discharge of patients from acute or community hospital wards to assess and manage rehabilitation/therapy needs, at the point that the patient is medically well enough to go home.

Early Intervention Vehicle (EIV) is a joint pathway, delivered with Herts County Council delivering crisis management to patients offering them a holistic assessment - physical, functional, and social all in one assessment.

We are a strong multi-disciplinary team and work in an integrated way with partner organisations to ensure holistic support is provided. We also have very strong connections with other community services within CLCH delivering care over shared pathways.
