Job summary
Homelessness is an increasing public health challenge impacting the health and well-being of children and their families placed in temporary housing. Children and their families are often displaced from their communities and, in particular, their schools. Boroughs can have multiple locations where homeless families are housed. The post holder will work in partnership to best meet the needs of homeless children and families seeking to reduce health inequalities and improve outcomes. To aim for the best possible input and outcomes for children in their first 1001 days and until they reach school age. This includes safeguarding children and young people identified as at risk of harm. To act as a resource for the mainstream 0-19 service providing expert advice and guidance. To act as an advocate for homeless families with relevant stakeholders but particularly with local authorities and MPs. To work to influence policies affecting the health and wellbeing of residents in homeless and temporary accommodation.
Main duties of the job
- Manage an enhanced caseload of families who are residing in temporary accommodation
- Providing the 0-5 Healthy Child Programme and 5-19 Healthy Child Programme, liaising with their named school nurse.
- Work in collaboration with local statutory and mainstream services to ensure that families become confident in accessing health and support services.
- Initiate, plan, and coordinate the care of children and families in consultation and collaboration with other professionals.
- Be aware of and act in accordance with CLCH's clinical standards.
