Job summary
Job Purpose
The One Care Home Team's purpose is to provide high-quality and safe re-active and pro-active face to face and virtual MDT support to care homes and their residents.
The post holder will manage a caseload of residents and will work in partnership with the care home team to deliver a consistent level of assessments, care and medical response to the residential and nursing homes within Barnet to meet the health care needs of the residents as part of the integrated care partnership function to assess, care plan, coordinate and deliver care for residents.
To provide targeted intervention for adults with complex health needs as part of the integrated care partnership.
To advocate for patients who face multiple barriers to accessing healthcare and raise awareness among other healthcare professionals, students and social care staff of the health issues affecting residents in care homes.
The post holder will be responsible for:
Managing a caseload of residents in care homes in Barnet.
Initiating and maintaining good communication networks.
Upholding best management and leadership skills in practice.
Maintaining professionalism at all times.
Main duties of the job
Managing a caseload of residents/patients in Care homes in Barnet.
To have oversight and named clinical lead for a portfolio of residents/patients in care homes within a PCN area.
To educate and support care home staff to provide high quality care for their residents, attending to the needs of adults who are particularly vulnerable and taking appropriate action as required.
When required, to undertake comprehensive assessment of the physical, psychological and social care needs of patients using the Single Assessment Process. The needs of carers should also inform this assessment.
To work in a collaborative manner with Primary Care staff, Acute services, Social Services and other agencies to ensure a seamless delivery of care.
To have knowledge of, and be able to effectively use local services and resources to promote patient care.

- RGN / RN with current registration
- Evidence of continuing professional education and training at degree level
- Community or Independent Nurse Prescribing qualification or prepared to work towards
- Qualification in team / service management or prepared to work towards
Desirable
- Advanced Physical Assessment course or equivalent
Experience

- Evidence of recent post-registration nursing experience in community nursing
- Experience of negotiating and liaising within a multi-disciplinary team
- Experience of teaching others and mentoring students
- Experience of co-ordinating and delivering care for people with complex, long-term conditions.
Skills & Knowledge

- Knowledge of principles of care for long term conditions, relevant NSFs and other national standards
- Detailed knowledge of Primary Health Care and collaborative working with other care providers, e.g. social care and voluntary services
