Job summary

The cardiorespiratory team delivers a range of services within Merton in a variety of settings. The team is multi disciplinary and made up of respiratory and cardiac nurses, physiotherapists, an occupational therapist and nursing assistant for a 12 month fixed term contract/secondment.

The Rehabilitation Assistant works as a key member of the team providing assistance and support to the patients and clinicians.

The post holder's main role is to support and assist the clinicians in delivering exercise classes and patient care in a variety of settings throughout the borough of Merton, including patient's homes, clinics and the local Leisure Centre. The Rehabilitation assistant supports the team to deliver excellent patient care and high quality clinical outcomes for Merton residents.

The successful applicant will be assisted to become proficient and competent in their role with training, education and clinical support. The team has weekly multi disciplinary meetings to share good practice, plan the workload and discuss patient treatment. The cardiorespiratory team prides itself in developing all team members to reach their full career potential and progress within the trust and the NHS.

Main duties of the job

To work under the supervision of the professional staff in the multi-disciplinary team for the care and rehabilitation of patients.

To contribute to the on-going assessment of a patient by completing the appropriate paperwork, recording and reporting all clinical and behavioural information to the nurse/therapist for action and planning purposes.

To perform nursing and therapy procedures where necessary for which adequate training has been given.

To be conversant with care plans of those patients with whom you work with directly prior to each intervention.

To follow rehabilitation plans independently without supervision, to review, update and report back within defined delegated responsibility framework.
