Job summary
An exciting opportunity has arisen within the Pembridge Specialist Palliative Care Service for a clinical nurse specialist to join our community team. We are looking to recruit a highly motivated and compassionate registered nurse with the skills and knowledge to fulfil this role. The successful candidate will be expected to demonstrate a high level of expertise clinically, have excellent communication skills and the ability to work flexibly with consideration to the needs of the service. This post participates in the CNS weekend working rotation.
The role is based at the Pembridge Hospice , St Charles Centre for Health and Wellbeing.

Main duties of the job
The post holder will be expected to provide expert symptom control advice, social, emotional and spiritual care, and support to patients and their families/carers. They will have responsibility as the community palliative care keyworker for their own patient caseload, working independently and ensuring patients have an individualised plan of care.
To actively participate in weekly Specialist Palliative Care multidisciplinary team meetings, attend GP meetings ,working closely with the wider community services to ensure excellent patient care.
