Main duties of the job

What You'll Do:As an Urgent Community Response Band 6 Nurse, you'll:

  • Provide high-quality, evidence-based care to patients experiencing health crises in their own homes.
  • Assess, plan, and deliver complex care interventions, including medication administration and wound care.
  • Work collaboratively with multidisciplinary teams to ensure seamless patient care.
  • Utilize innovative approaches to improve patient outcomes and efficiency.
  • Adhere to strict safety protocols, including lone working guidelines.

What We're Looking For:

  • Registered Nurse with a valid NMC PIN
  • Strong clinical skills and experience in community or acute care settings
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Passion for delivering exceptional patient care

  • Previous experience of undertaking comprehensive health needs assessment and writing care plans
  • Demonstrate knowledge of the purpose and value of supervision.
  • Experience of teaching of junior staff / students, peers in-service training setting.

  • Experience of teaching, patient and public involvement and health promotion in practice
  • Experience of providing acute care response to frail and elderly patients in acute or community setting
