Job summary

Nurse Advisor, Trusted Assessor (Barnet Planned Care- One Care Home Team- CLCH NHS Trust) Permanent post 30hours/week.

We are seeking a motivated, compassionate and dynamic nurse with experience in comprehensive health needs assessment and discharge planning to apply for this exciting opportunity to work as a Trusted Assessor in the One Care Home team.

The One Care Home Team's purpose is to provide high-quality and safe re-active and pro-active face to face and virtual MDT support to care homes and their residents.

The post holder will work closely with colleagues from the acutes and care home managers within the borough of Barnet. You will have the chance to rotate in the two hospitals the TA is covering.

We offer regular clinical supervision, guidance and training for personal and professional development in a friendly and supportive environment. This includes a program of in-service training and other training and development opportunities.

Main duties of the job

The post holder will be responsible for:

  • Working within acute hospitals to identify patients with complex care needs to support and facilitatetimely and appropriate safe transfer of care following discharge from hospital to avoid unnecessarydelays.
  • Acting as a point of contact for Care HomesRegularly communicating with a range of stakeholders including hospitals, care homes and community teams about discharge arrangement.
  • Providing support to Care Home and Hospital staff relating to admission and discharge processes andalso clinical skills where applicable (including supporting the identification and arrangement of requiredtraining

  • oProfessional qualification i.e. Registered Nurse, Social Worker or Allied Health Professional

  • oComprehensive knowledge of project management and/or health information systems development
