Job summary

Harrow Rapid Response aims to prevent unnecessary hospital admissions by providing urgent and high-quality care in a community setting.

Patients are referred to the team from primary care, London Ambulance Service, local Accident and Emergency departments and other community services.

Within 2hrs from time of referral the clinicians aim to obtain a detailed history from patients, families, or carers, and complete a comprehensive physical assessment. The findings of the assessment and other diagnostic tools like blood results are then used to formulate a diagnosis and provide treatment. If required, the patients are closely monitored through follow-up home visits where clinicians need to be able identify deteriorating patients and act on such findings.

Apart from blood tests the service can also request certain scans including Duplex for DVT and x-rays in a local hospital. In this hospital, Rapid Response Service also supports the Accident & Emergency department to facilitate the process of safe discharge and preventing admissions.

Main duties of the job

Rapid Response aims to provide high standards and quality of care whilst ensuring evidence-based practice and respecting patients preferences and wishes which could be impacted by religious and cultural beliefs.

We are looking for practitioners who are passionate about providing excellent patient care and continuous development of the service through quality improvement. You will work as an autonomous practitioner and will be given the opportunity to gain additional skills such as venepuncture and catheterisation training. This role will give you an opportunity to expand your clinical and leadership skills.

The candidate must hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)

The service operates Monday to Sunday from 8am to 8:30pm, with opportunities for on-calls until 10:30pm. When working full-time you will be allocated 13 shifts across a 4-week rota period.
