Job summary
The service delivers high-quality care to patients in their own homes, primarily preventing hospital attendance/ admission and facilitating early discharge from the hospital. It operates from 8 a.m. to 22:30, seven days a week.
JOB PURPOSE
The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Team, other health care professionals, and statutory and voluntary agencies to deliver excellent patient care.
The post holder will be required to:
- Work on a rotational shift basis within the Brent Rapid Response Team - service operating hours 8 am - 10:30 pm 7day a week.
- Be the Team Leader for designated shifts on a rotational basis responsible for day-to-day management of the service.
- Take designated responsibility for discrete areas of operational management/ clinical practice.
- Prioritise and manage own workload with minimal supervision.
Main duties of the job
- As a Senior Nurse, I provide operational management and clinical leadership for the Rapid Response Service, including triage of referrals and ensuring efficient and effective resource use via daily staff timekeeping, work allocation, and rest breaks. I also respond to immediate sickness absences (requesting cover as per protocol as required).
- Utilise advanced clinical and diagnostic skills and expert knowledge to conduct thorough assessments of physical and psycho-social needs to develop a comprehensive short-term care plan to optimise health and wellbeing that will prevent hospital attendance/ admission or allow early discharge home.
- Provide direct, advanced, autonomous clinical care to patients with undifferentiated, undiagnosed, complex health needs and long-term conditions.
- To ensure that policies, procedures, clinical governance arrangements and lines of accountability are adhered to by the Rapid Response Team to ensure safe and effective clinical service delivery.
- Monitor standards and performance of the team in line with clinical governance and CLCH objectives.
- Be responsible for your own professional development through active learning and reflective practice.
