Job summary
The post holder will be required to:
- To provide highly skilled and autonomous care to a designated caseload including taking responsibility for the assessment, management, and evaluation of evidence-based nursing care to patients in a variety of community settings.
Act as the Team Leader for the Rapid Response Nursing Team on a rotational basis responsible for day-to-day management of the service; acting as a key point of contact for GPs and other general practice staff.
To work within and across professional and organisational boundaries, liaising with GP's other professionals and statutory and voluntary agencies to deliver individualised and expert nursing care to a high standard to client, and leading communication with all stakeholders.
Take designated responsibility for discrete areas of operational management/ clinical practice.
Work on a rotational shift basis - 8am - 4pm and 2pm - 10pm, when working on the LAS/UCR car shift is 8am - 6pm all services are 7 day a week including bank holidays.

Main duties of the job
To provide skilled, effective, and evidence-based nursing care to patients with complex care needs, in a variety of community settings.
Utilise advanced clinical and diagnostic skills and expert knowledge to carry out thorough assessments of physical and psycho-social needs to develop a comprehensive short term care plan aiming to optimise health and wellbeing that will prevent a hospital attendance/ admission or allow early discharge home. To include the needs of carers.
Provide direct advanced, autonomous clinical care to a range of patients presenting with undifferentiated, undiagnosed and/ or complex health needs and long-term conditions.
Work within Clinical Guidelines, Policies, Procedures and Patient Group Directions (PGD's) utilising expert judgment to work outside these frameworks where necessary.
Prescribe as an Independent Nurse Prescriber as required, within professional competence, ensuring that all duties in relation to medicines management meet required professional and CLCH standards.
