Job summary
Community Matron (Barnet Planned Care- One Care Home Team- CLCH NHS Trust).
We are seeking a motivated, compassionate, and dynamic nurse with experience in care of the elderly and long-term conditions to apply for this exciting opportunity to work as a Community Matron in the One Care Home team.
The One Care Home Team's purpose is to provide high-quality and safe re-active and pro-active face to face and virtual MDT support to care homes and their residents.
The post holder will manage a caseload of care homes and will work in partnership with the care home team to deliver a consistent level of assessments, care and medical response to the residential and nursing homes within the borough of Barnet.
We offer regular clinical supervision, guidance and training for personal and professional development in a friendly and supportive environment. This includes a program of in-service training and other training and development opportunities.
Main duties of the job
- Managing a caseload of care homes in Barnet.
- Initiating and maintaining good communication networks.
- Upholding best management and leadership skills in practice.
- Maintaining professionalism at all times.
- Participating in teaching and training.

- RGN / RN with current registration
- Post -registration qualification in district nursing or equivalent
- Evidence of continuing professional education and training at degree level
- Qualification in team / service management or prepared to work towards.
- Mentor or practice based learning training
- Community or Independent Nurse Prescribing qualification or prepared to work towards
Desirable
- Advanced Physical Assessment course or equivalent
