Job summary
Central London Community Health are excited to announce the opportunity to employ GPs on a sessional basis/, within an expanding team. This new, exciting, and challenging role offers the opportunity to join a team of GP's, pharmacists, dietician, senior nurse clinicians, healthcare coordinators and care assessor, in the ongoing development of the Brent Health Matters Flagship Model for health inequalities.
No two days will be the same; you will have patient contact, be expected to work with and support the multi-disciplinary team, assessing both health and social needs through holistic problem solving and you will have the opportunity to shape the service we deliver.
This is an exciting role for someone with an interest in health inequalities and able to understand the current barriers affecting our population group and adapt your consultation skills accordingly, along with utilising your existing knowledge of Brent and experience of working in Brent with Brent GPs.
In this role, you will be required to liaise with the registered patient's GP for patients who seldomly access health care, provide professional support to the services clinical pharmacists on medication care plan reviews, clinically review and advise on abnormal investigations, medical escalations and supporting the large scaled community events that are offered as part of the wider Brent Health Matters programme delivery.
Main duties of the job
Respond to medical problems presented by patient's or the clinical team, including history taking, examination, supporting diagnosis and requests for referrals, where appropriate, to the registered GP
To provide clinical oversight to the team including reviews of the services Quantitative and Qualitative KPI's.
To assist and support relationship building with key stakeholders that may use or refer into the service
To provide clinical GP support, feedback and strategies at agreed BHM community events.
Conduct clinical reviews with patients, in their homes or at a community / clinic setting
Provide a weekly board round to discuss case managed patients that may require medical interventions.
Liaise with the registered GP's to identify high risk patients following weekly ward rounds, for appropriate follow-up at the patient's practice or within the community setting
