Job summary
To provide in conjunction with the Health Visiting team an accessible, person centred, effective needs led public health nursing service to the 0-5 age population and their families. To participate in service development and health improvement activities. To work in collaboration with other health care and service providers, including Children centres and Early years settings to identify and address health needs. To monitor, maintain and develop the service. To deliver the role specific elements of the Universal Healthy Child Programme (DH 2009)

Must hold previous experience working as a Community Staff Nurse in the 0-19 team
Main duties of the job
To organise and prioritise own work without direct supervision, reporting any issues, concerns or personal limitations to the Team Lead.
To be responsible for the care and safe keeping of equipment issued for personal use and to report any defect or loss.
To facilitate and maintain a good working relationship with GP's, Children Centres, Early Years Providers and other health and social care service providers, so as to provide a comprehensive service to children and parents.
To attend and actively participate in team, locality and other relevant meetings.
