CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

Our Unplanned Care Services aim to prevent hospital admissions and support people to get out of hospital quickly. Our friendly teams are made up of skilled nurses, paramedics, therapists, GPs, technical instructors, health care assistants and administrators. We have an innovative staffing model that includes nurse consultants and advanced practitioners, which means we're able to keep very unwell patients at home safely, and the service operates 7 days a week 8am to 8pm.

Main duties of the job

Our services have grown exponentially since they started in June 2020 and we currently provide:

- Rapid Response where we aim to respond to urgent calls in 2 hours- Discharge to Assess Therapy (DTA) - where our teams provide therapy to patients at home and in care homes.- Early Intervention Vehicle (EIV); which is a vehicle that mostly responds to people who have fallen over at home and is jointly delivered with our adult social care colleagues at Herts County Council- Hospital at Home - (Virtual wards) which are jointly and closely delivered with acute colleagues. We work closely with acute colleagues to care for patients in their own homes who previously would have had to be admitted to hospital- we are also co-located with the Prevention of Admission adult social care team,

We are expanding our leadership model in the team and are now looking to recruit a Band 7 Team Lead to support the Clinical and Operational Lead in operationally managing these services. The role is varied, and a great opportunity to develop your management/leadership skills. It presents a perfect opportunity for those who are eager to progress in their leadership journey within the NHS, offering in-depth experience of collaborative and integrated working and of leading in a complex and ever-changing clinical environment. As such it would present an excellent springboard into more senior clinical and operational leadership roles.

To lead others from a base of clinical credibility in order to model and support improved practice, integrity and partnerships. To provide supportive and facilitative leadership to the team and colleagues. Manage the day-to-day, on-going delivery and development of the service within remit ensuring delivery of a service that is responsive, high quality, value for money and meets contractual obligations. Responsible for service delivery with the available resources, and monitoring of performance against, set targets (activity, finance, outcomes, utilisation) Maintain own accurate records and statistical returns and develop, maintain and monitor the effective and timely record keeping processes used by all team members including the accurate entering of data onto the Trusts clinical systems within required time frames and ensuring that qualitative information is collected in the designated format.
