We are looking to recruit a Band 7 DSN to join our dynamic team.

We are a multidisciplinary team consisting of Consultants, Dietitians, Podiatrist as well as Specialist Nurses. The team receives referrals from many primary care and community services as well as local acute hospitals.

We are a unique community provider as we manage people with both Type 1 and Type 2 diabetes and are an initiating centre for Freestyle Libre. We also deliver DAFNE & DESMOND structured education.

We are based in Morden (at the end of the Northern line) and work within the London Borough of Merton. Morden Underground station is just around the corner and Wimbledon Station 1.5 miles away. We are also on many bus routes, with a tram station very nearby.

Main duties of the job

  • Relevant post registration training and experience in the management of patients with Type 1 & Type 2 Diabetes.
  • Ideally be an Independent Nurse Prescriber or demonstrate a willingness to achieve this qualification.
  • Be a great a team player who is willing to work collaboratively with the multi-disciplinary team to deliver the best care possible for our patients.
  • Have a strong commitment to the delivery of high quality, safe, patient care and be a good role model in reflecting the Trusts core values.
  • Be competent to offer guidance and supervision to members of the clinical team

Job description

Job responsibilities

  • To be responsible for providing a specialist clinical diabetes service to clients with diabetes on oral hypoglycaemic agents, insulin or insulin conversion and other new therapies, including appropriate counselling and lifestyle advice in various community settings such as GP practices, health centres and clients homes.
  • To work autonomously to provide expert advice to clients, carers, and clinicians. This will involve assessment and the development of treatment plans using evidence-based outcomes and involving patients throughout the process.
  • To use professional judgement and clinical expertise to independently interpret and analyse complex situations and information to formulate treatment plans according to evidence based practice and modify according to changes in clinical condition.
  • To be clinically competent to give specialist diabetes advice, treatment and counselling to support behavioural change, in adults with complex and diverse healthcare needs across health and social care.
  • To provide expert advice to medical practitioners in virtual clinics, by email or phone queries.

  • Current registration with the NMC, Showing a registered General Nurse.
