Job summary

CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

An exciting opportunity has arisen for a Band 4 Cardiac Rehabilitation Assistant Practitioner to work in the community Cardiac Rehabilitation service in West Hertfordshire. We require a highly motivated and experienced BACPR exercise instructor or an exercise instructor with a background in working with specialist health populations.

The post holder must have excellent verbal and non-verbal communication skills and must be confident in working across professional boundaries to join our enthusiastic multi-disciplinary team. You will need to be flexible, innovative and a good team player with the ability to work as part of the multidisciplinary team within the community setting.

Main duties of the job

The successful individual will be responsible for providing a range of exercise assessments and delivering with support from the wider multidisciplinary team cardiac rehabilitation exercise sessions for patients following a recent cardiac event, cardiac procedure or with a diagnosis of heart failure. These exercise assessments and group therapy intervention will take place in a variety of community venues and virtually.

  • To support an efficient day-to-day running of the cardiac rehabilitation service by providing highly skilled support work and delivering the cardiac rehabilitation exercise component of the program.
  • To contribute to the general running of the cardiac rehabilitation service; inclusive of all formats of cardiac rehab delivery i.e. face to face, virtual and home based cardiac rehabilitation.
  • To assist in the overall clinical and administrative management of patients under the cardiac rehab services care.

Essential

  • Registered Exercise Professional - Level 2 & 3 exercise instructor
  • Membership of relevant professional body and / or membership of special interest group (ie BACPR / CIMSPA/ BASES)

Desirable

  • Level 4 BACPR cardiac rehabilitation exercise instructor qualification

Experience

Essential

  • Understand the role of cardiac rehabilitation for individuals with cardiac health conditions
  • Basic knowledge of cardiac health conditions
  • Understanding of the concept of rehabilitation/recovery within the clinical area
  • Experience of working as part of an MDT
  • Experience of delivering exercise interventions in a variety of settings; community, patient homes and virtually

Desirable

  • Experience of working with cardiology patients
  • Experience prescribing exercise for therapy specific interventions such as cardiac and pulmonary rehabilitation
  • Experience in delivering cardiac rehabilitation exercise classes
  • Previous experience of working effectively with service user/patient undergoing cardiac rehabilitation e.g. phase IV exercise programme

  • A working knowledge of clinical governance and evidence-based practice
