We are looking to recruit a dynamic, motivated and innovative Pharmacist to join our friendly and approachable Medicines Management team at CLCHT. The postholder to work predominantly in North Central London division but will also be required to travel to other Bedded Services sites within the organisation so will require flexibility. The base site will be Finchley Memorial.

Main duties of the job

We are looking for a full time or a part time Pharmacist to provide a Clinical Pharmacy service to our inpatients in our Rehabilitation Service across the Trust to the London sites.

CLCH is one of the largest Community Health Trusts in England and provides services across 11 CCGs to more than two million people across tenLondon boroughs and Hertfordshire. We are part of 4 Sustainability and Transformation Partnerships (STPs). With the aim of shifting health care from acute hospitals to settings closer to people's homes and moving away from reactive care to preventative care based on early interventions.

As an integral member of the team, you will be providing specialist expertise in the use of medicines whilst supporting to address both public health and social care needs of patients. You will be involved in conducting structured medication reviews for patients with polypharmacy, long term conditions or high risk patients ensuring an integrated approach to the management of medicines.

If you are successful, you will join the existing team of Band 7 Pharmacists at the Trust covering a range of rehabilitation specialities. You will be supported by experienced Senior Clinical Pharmacists allowing you to build new skills and have the opportunity to develop strategic, quality improvement and leadership skills.

  • Vocational Masters Degree in Pharmacy
  • Professional registration with the General Pharmaceutical Council (GPhC).
  • Postgraduate Clinical Pharmacy Certificate qualification or equivalent

Desirable

  • Postgraduate Clinical Pharmacy Diploma qualification or equivalent
  • Independent prescriber
