Job summary

Central London Community Healthcare NHS Trust are looking to recruit 2 dynamic, skilled, and motivated pharmacy technicians to work in Balham, Tooting, and Furzedown Primary Care Network (PCN). The postholder will work alongside an established team of pharmacists as well as GPs, practice staff and other healthcare professionals (HCP) to provide support on medicines optimisation services within the PCN. In addition to this be supported by the technician team within CLCH.

Balham, Tooting, and Furzedown PCN is a forward-thinking PCN comprising of seven GP Practices with a total patient population more than 56,000. They have a team of existing pharmacists and are wishing to expand the team further, as they recognise the value the pharmacy team bring to the practices and patients.

Main duties of the job

The postholder must be a qualified Pharmacy Technician, registered with the General Pharmaceutical Council. You must be able to demonstrate good organisational and problem solving skills. Previous experience of working as a qualified, registered pharmacy technician in primary care or community is desirable. A knowledge of medicines management issues faced in primary care is essential with excellent decision-making and problem solving skills. Additionally, compassion, flexibility, excellent communication and time management skills are essential along with the ability to build relationships with various health and social care professionals.

To undertake medicine optimisation tasks including effective medicines administration (e.g., checking inhaler technique), supporting with identification of patients suitable for a structured medication review e.g., frail, recent discharge, history of falls, medication reviews, medicine reconciliation and medicine queries.

Utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively, through shared decision-making conversations with patients.

Support, as determined by the PCN, medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings and linking with local community pharmacists.

Assist in the delivery of medicines optimisation, management incentive schemes and patient safety audits.

To support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. This will be achieved through undertaking clinical audits (e.g., use of antibiotics), supporting quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services both locally and nationally.

Working alongside PCN Pharmacists to ensure safe and robust medicines management processes for patients within the PCN.
