Specialist Speech and Language Therapist- Integrated Community Stroke Service (ICSS)/NETT - B6 - Harpenden Memorial Hospital

Come and work with us in West Hertfordshire. We have recently received a significant investment into stroke services enabling us to offer new posts Integrated Community Stroke Service (ICSS) service follows the national blueprint for the future of stroke services, with multidisciplinary teams offering stroke rehabilitation at a range of intensities. This includes:

  • Early Supported Discharge (ESD): high-intensity rehabilitation for up to 6 weeks following discharge from hospital for people with mild-moderate needs following a recent stroke.
  • Neurological Enhanced Transition Team (NETT): high-intensity rehabilitation for up to 6 weeks following discharge from hospital for people with moderate to severe needs following a recent stroke, new onset or recent change in a neurological condition.
  • Community Stroke Service: lower intensity, longer term rehabilitation and support to optimise health and psychological wellbeing over the longer term for people and families living with stroke

The ICSS provides assessment and rehabilitation for adults, from 18 years upwards, who have had a Stroke in both home and out-patient clinic settings. We work closely with other healthcare professionals, services and agencies from across the whole Stroke pathway and wider CLCH adult community services portfolio.

Main duties of the job

Working within a cohort of SLTs, this new post creates a great opportunity for someone who is conscientious and adaptable to deliver forward thinking, progressive stroke rehabilitation to patients primarily in their own home, but also in a clinic environment. The successful candidate will have excellent communication skills and be able to demonstrate an adaptable approach whilst meeting guidelines and stroke standards.

You will form an essential part of a dedicated, knowledgeable multi-disciplinary team that regularly engages with team and individual clinical supervision.

Development and training opportunities are available through the CLCH Academy.

You will maintain responsibility for an individual clinical caseload within the wider ICSS team supporting people with communication and swallowing difficulties. There are opportunities to be involved in developing the newly enhanced service, including delivering group interventions.

Patients are seen in their own homes or community settings, with a recently renovated office and outpatient base at Harpenden. All team members are fully agile working, enabling blended in-person and virtual sessions to be delivered as best meets client needs. As such, the postholder will be able to work flexibly from home or the office in a pattern negotiated according to their circumstances and service needs.

  • To undertake multidisciplinary neurological assessments, to be able to identify the needs of the clients and to work closely with the MDT on joint goals. These clients will present with a variety of complex neurological conditions, but with an emphasis on those recovering from stroke.
  • To analyse auditory, visual and kinaesthetic aspects of clients communication and to identify appropriate strategies to facilitate and enhance communicative effectiveness in the home, at work and other relevant settings
  • To carry out specialist neuro-physical examination of the orofacial process including the mechanism of eating and swallowing

  • Recognised Speech and Language Therapy Degree Qualification or equivalent
  • License to Practise from the Royal College of Speech & Language Therapists.
  • Evidence of registration with the HCPC
  • Basic Dysphagia Training or equivalent experience
  • Evidence of successful completion of specialist short courses relevant to speech/language/communication issues for adults

Desirable

  • Counselling skills training.
  • Other appropriate courses relevant to the case-load
