We want a motivated and innovative Pharmacist to join our expanding Medicines Management team at Central London Community Healthcare Trust Hertfordshire Division .

You will join a team that will give you opportunities to implement your skills and knowledge and develop them further to enhance the services provided.

We are part of 4 Integrated Care Boards with the aim of shifting health care from acute hospitals to settings closer to people's homes and moving away from reactive care to preventative care based on early interventions.

Main duties of the job

You will lead and support the Herts Medicines Management Team - to be flexible, plan and organise own workload as well as developing and supporting the junior staff.

If necessary and depending on development and service needs, you may need to work in different localities across the Trust. As part of an integrated team you will help to ensure high standards are maintained within our Community Health Services and Bedded Services.

You will support medicines management governance work streams in reviewing policies and procedures and to review risk management strategy a well as providing professional expertise, advice on governance and patient safety whilst supporting the delivery of a high quality clinical pharmacy service and medicines optimisation service to patients across services within Herts.

There is no weekend working and on call cover is provided remotely.

  • Vocational Masters Degree in Pharmacy
  • Professional registration with the General Pharmaceutical Council (GPhC)
  • Postgraduate Clinical Pharmacy Certificate qualification or equivalent
  • Postgraduate Clinical Pharmacy Diploma qualification or equivalent
  • Independent Prescriber or working towards qualification
  • Takes personal responsibility for Continuing Professional Development as evidenced by portfolio
  • Uses reflective practice for personal and professional development

  • Post-qualification experience of clinical pharmacy, medicines information and prescribing advice
