An exciting new opportunity for an experienced Band 2 or 3 to join the newly restructured Harrow Integrated Care Service.
The Harrow Integrated Care service provided services to Harrow CCG and is split into three locality teams. This post will be located in one of the Harrow localities. The post holder will work closely with the team leads, clinicians to develop the service and maintain close working relationships with both acute and community services.
Main duties of the job
We require an organised registered nurse who can demonstrate clinical skills in community Nursing to work at band 6 level. The ability to work alone in a community multidisciplinary team and support junior staff is essential to this role

- The Therapy Assistant will provide a mixture of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional. The Therapy Assistant will work in a variety of settings including community centres and patient/carers own homes.
- The post holder will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The Therapy Assistant will promote optimum independence and individualised client centred care to a delegated caseload.
- The post holder will also support the Integrated Team to deliver against key performance indicators and maintain quality of the service.

- Basic skills in literacy Level 1 and numeracy level 3
- Diploma NVQ Care level 2 or equivalent experience.
- Must undertake Diploma NVQ 3 care
- All new Healthcare Support Workers across the NHS are required to complete the Care Certificate programme. Newly recruited HCSW's are identified during our recruitment process but line managers have the responsibility to make sure that new support workers working within their teams are enrolled on the programme. Completing the Care Certificate ensures that support workers have the required values, behaviours, competences and skills to provide high quality compassionate care.
