You will form an essential part of a dedicated, knowledgeable multi-disciplinary team that regularly engages with team and individual clinical supervision. Main duties of the job
You must be a car owner and have a valid UK driving license as this role involves working with patients in their own home, covering West Hertfordshire.
Following significant investment into community stroke services for Herts Valleys, we are now welcoming applications from enthusiastic Occupational Therapists with an interest and, ideally, experience in stroke rehabilitation to work collaboratively within the new Integrated Community Stroke Service (ICSS) offering both community and discharge pathways.
This modern stroke service infrastructure follows the national service model and is delivered by a multidisciplinary team offering stroke rehabilitation, in patients own homes in and around West Hertfordshire, at a range of intensities. Within the model we have the following:
- Early Supported Discharge (ESD), commonly a course of high-intensity rehabilitation that a patient receives over up to 6 weeks following a discharge for mild-moderate new stroke.
- Neurological Enhanced Transition Team (NETT), commonly a course of high-intensive rehabilitation delivered up to 6 weeks following a discharge for moderate to severe new stroke or other neurological conditions.
- Community referral, which is for patients experiencing a change in their daily function directly related to a pre-existing stroke injury.

- To be an active member of the CLCH ICSS multidisciplinary team, providing a comprehensive occupational therapy service to residents within West Hertfordshire and in their own homes. The ICSS includes Early Supported Discharge Team (ESD) Neuro Enhanced Transition Team (NETT) and Community Stroke team.
- To work within professional standards and clinical guidelines, promoting best practice and undertaking all aspects of clinical duties as an autonomous practitioner, including assessing and treating own caseload of patients/clients and maintaining professional documentation.
- To build up and maintain key relationships with referrers including GPs, acute providers, Bedded Services, District Nurses, Wheelchair Services and other services across CLCH to facilitate appropriate referrals. To have an understanding of the occupational therapy role within the multi-professional team.
- To be responsible for the supervision of rehabilitation assistants and occupational therapy students.
- To work mainly in the patients homes but also in a variety of settings including clinics, residential care homes, continuing care homes and community centers.
- To support personal and professional development through active participation in Continuing Professional Development (CPD) and by participating in any audit or research being undertaken by the service.
