Job summary

CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

An exciting opportunity has arisen for a Band 7 Occupational Therapist to work in the Long Covid Rehabilitation and Assessment service in West Hertfordshire.

We require a highly motivated and experienced Occupational Therapist with a background in working with specialist health populations in fatigue, cognitive and vocational rehabilitation. The post holder must have excellent verbal and non-verbal communication skills and must be confident in working across professional boundaries to join our enthusiastic multi-disciplinary team. You will need to be flexible, innovative and a good team player with the ability to work as part of the multidisciplinary team within the community setting.

Main duties of the job

The successful individual will be responsible for providing a range of assessments and delivering with support from the wider multidisciplinary team long covid rehabilitation.

You will be joining a friendly supportive team where professional guidance is readily available and continuous professional development is strongly encouraged.

Job description

Job responsibilities

  • To clinically work with the population of patients who developed Covid19 and where either admitted to hospital or were discharged from A&E home under the virtual Covid Hospital. This will require performing virtual assessment / screening, following advised pathways, creating referalls, liaison with other services primary care, clinical, social aswell as attendance at MDTs.
  • To assist the 8a in managing the cohort of patients involved, data entry, analysis, assisting in writing reports
  • To work autonomously to deliver specialist clinical care appropriate to the needs of the patient group and based on principles that are evidence based and which enhance the quality of patient care.
  • To undertake comprehensive virtual assessments of complex patients with co-morbidities, and to develop treatment plans for patients and to formulate individualised management and treatment plans using clinical reasoning.
