Job summary

Are you what we are looking for? Do you consider yourself to be the nextgeneration of leaders in the NHS, If so an exciting opportunity has arisen for a Band 6 Nurse in the Outer North West Division for a community nurse to work closely within the planned care of Nursing and Therapy. This is a permanent post.

This is a great opportunity to develop your understanding of quality drivers and to work closely with clinical teams to support the delivery of high-quality care and positive patient and staff experience.

We are looking for a dynamic and motivated individual with nursing experience who can provide local clinical leadership for the multi-disciplinary team working across the Division.

The successful candidate will have the opportunity to further develop quality expertise and lead key projects and pieces of work by developing strong relationships and network across the Division. You will work collaboratively with the wider multi-disciplinary team, other health and social care practitioners and the nursing staff, to provide the best care and quality outcomes for patients and service users, in-keeping with national guidelines.

The clinician must demonstrate strong clinical skills and experience working in and understanding Community services. They need to be able to evidence a broad understanding of the quality agenda and have experience of leading teams and services through change and development.

Main duties of the job

To set and maintain standards of care and deploy, co-ordinate and supervise the nursing team and learners as required.

To provide leadership and management to a team of community nurses and other staff covering a specified locality.

To be responsible for the team's assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8am-10pm

To be responsible for the team's maintenance of patient and other records

To participate in the long term management of patients within the locality

To have continued responsibility for a defined caseload.
