CLCH Community Planned Care Services are currently seeking a motivated Night Senior Nurse band 6 to join our community based service in London Borough of Barnet.

Barnet Planned service provide community Nursing services to Barnet CCG and is split into three locality teams. This post holder will be covering one of the permanent post within Barnet night nurses team 34.5hrs weekly, working 3 nights per week from 20.00 to 08.30am. Covering all the patients across London Borough of Barnet. The post holder will work closely with the locality manager, team leads/ other clinicians to develop the service and maintain close working relationships with both acute and community services. We are looking for a nurses who already have experience in the community nursing with a wide range of clinical skills. The post holder must have experience is syringe driver, catheter care and complex patients management.

Essential: It is essential to have a current clean driving licence (less than 6 points) and have access to your own car. Please note that without clean driving licence and access to your own car your application will be rejected.

You need to be able to use a map and SAT NAV to navigate to patient's homes.

  • To provide skilled nursing care working as part of the Primary Health Care Team to patients in a community setting across the 3 localities of district nursing team in Barnet between the hours of 20:00 and 80.30, including weekends and band holidays.
  • To actively monitor, maintain and develop the service provided.
  • To take responsibility for managing the caseload and the team, with the support of the locality manager, to provide higher quality care for patients already on district nursing caseload that requiring a rapid response visit during the out of hours call.
  • To deliver a proactive case managed approach for patients with long term care needs, to support palliative/EOL care patients and their carers by providing advice and symptoms control; and to support and care for patients, referred by 111, the GP out of hours service, LAS, requiring catheter care as well as more urgent care.
  • To actively participate in service development activities, MDT meeting when requires and team meetings.
