Job description
Job responsibilities
Community services provides invaluable care to people in their own homes, care homes, or close to where they live, in clinics and GP practices.
Main responsibilities will be dependent on Grade and speciality.
Which community services do we run?
In recent years, helped by Covid, healthcare services are increasingly being provided away from Acute Hospital sites in favour of care in community clinics or at home. CLCH is proud to provide a range of community adult healthcare services including:
- Bed based (community inpatient) rehabilitation services, including neuro rehab
- Community Neurological service,
- Community (district) nursing teams,
- Community physiotherapy and occupational therapy / domiciliary rehabilitation teams,
- Bladder and bowel service,
- Cardiac rehabilitation,
- Leg ulcer service,
- Lymphoedema,
- Integrated community stroke service,
- Community heart failure service,
- Podiatry,
- Rapid response service,
- Specialist palliative care,
- Speech and language therapy,
- Tissue viability,
- Sexual health services
- Respiratory Services
These services are all supported by our Single Point of Access and Administrative support teams, and we could not do it without them!
We are continually developing services and designing new clinical pathways. In the last couple of years for example we have been working alongside the Clinical Commissioning Group (now Integrated Care Board) and local Acute Trusts to develop rapid response/prevention of admission services, virtual wards, covid vaccination programmes, and long covid services.
We have also had the opportunity to develop new models of care in partnership with local voluntary sector organisations, and we really want to build on this in the future.
There is always more to do and we are looking for passionate people, who really want to make a difference to people in our local community, to help us to deliver services and improvements to take us from Good to Great.
