Job summary

Jade ward is a general and stoke rehabilitation adult inpatient ward in Edgware Community Hospital. We have dedicated rehabilitation Multi-Disciplinary Teams consisting of Registered Nurses, Occupational Therapists, Physiotherapists, Dieticians, Speech therapist, Pharmacists, Activities Coordinators, Rehab Support Workers, Care assistants and Housekeepers. As a team, the aim is to support patients to regain function, confidence, self-respect and much more.

Main duties of the job

We are looking for a staff nurse to be part of the team in a challenging environment and to remain focused on enabling staff to deliver the Trusts core objectives of high quality patient care whilst supporting and developing the individuals within the team. The individual will need to be focused on the patient experience to ensure all patients will have a positive and comfortable stay in the ward. You will need to display excellent communication skills, and offer care in a kind and compassionate manner. You will be dynamic, innovative, hardworking, and have excellent written and verbal communication skills. Job description

Job responsibilities

To monitor length of stay to support effective discharge processes working in collaboration with clinical teams in order to ensure there is a defined pathway of care for a patient including timely transfer/discharge liaising with external agencies appropriately.

To ensure that the ward has the correct staffing levels.

To be aware of and champion current trends and developments in nursing and in the NHS such as improving productivity and reducing length of inpatient stay.

Ensure & support team awareness of effective bed co-ordination.

To take a key role in the induction, training and mentorship of nursing staff, and provide clinical supervision as appropriate.

To work with the Modern Matron and Clinical Lead for inpatient rehabilitation to secure and assure the highest provision of care by providing strong leadership, act as a role model and resource, encouraging and developing the team to sustain and implement best practice within the unit.

Facilitate multi-agency, multi-disciplinary integrated care working ensuring patients needs are met.
