Job summary

We have an exciting opportunity for you to develop your skills in an innovative and established community dietetic role. We are seeking a dietitian with good clinical knowledge and a commitment to preventing unnecessary hospital admissions. This role will provide you with a wide portfolio of experience in both nutrition support and chronic disease management and the opportunity to be part of highly skilled multidisciplinary team.

Managing your own clinical caseload, you will undertake domiciliary visits to patient's own homes and in care homes (both virtually and face to face) and also provide outpatient clinics.

Two days per week will be spent with the Rapid Response Team, with the focus on visiting patients at home who are at high risk of hospital admission and require urgent clinical input. The successful applicant will be working within a dynamic multidisciplinary team, incorporating nurses, physiotherapists, occupational therapists, speech and language therapists, dietitian, paramedics, discharge coordinators, consultant physicians, rehabilitation technicians, health care assistants and a pharmacist.

Main duties of the job

You will be joining an experienced, supportive and award-winning team of community dietitians who hold regular peer support meetings and provide 1:1 supervision to facilitate clinical excellence and personal development. We actively participate in audit, student training and encourage research. You will have the opportunity to undertake hybrid working as part of your role.

You must be able to demonstrate skills and enthusiasm for working in a community setting. Communicating evidence-based personalised dietetic advice in an engaging style is a key component of this position.

This is a 0.8 WTE (30 hours per week) fixed term maternity cover Band 6 position. The post is funded until 30.07.2024.

Qualifications

Essential

  • Degree or postgraduate diploma in Nutrition & Dietetics
  • Health and Care Professions Council registration
  • Evidence of continuous professional development

Desirable

  • Membership of British Dietetic Association
  • Member of Special Interest Group relevant to this post
  • Post graduate qualification in dietetic related field (eg health promotion, Cognitive Behaviour Therapy).

Experience

Essential

  • Significant post-registration experience in a range of clinical areas of dietetics
  • Experience in managing patients with nutrition-related conditions such as malnutrition, diabetes, obesity & coeliac disease (including patients with mental health needs or learning disabilities)
  • Experience in training / teaching healthcare professionals and patients
  • Experience of providing dietetic advice to nutrition support patients
  • Experience in contributing to policy, procedures, guidelines and clinical standards developments within the service

Desirable

  • Experience of providing home enteral tube feed dietetic advice
  • Experience of working in the community setting
  • Experience in supervising the work of others
  • Experience of using SystmOne, RiO, EMIS Web or similar systems
