We have an exciting opportunity for a Band 6 Dietitian to join our Nutrition and Dietetics Team.

The community dietetic service includes a successful and proactive team of dietitians and dietetic assistant practitioners covering the three inner boroughs of London (Westminster, Kensington & Chelsea and Hammersmith & Fulham). We are passionate and strive to bring high quality evidence base dietetic practice to the people who use our services. Main duties of the job

This post is ideal for someone who isenthusiasticandaself-motivated individual,withan interest in developingtheir knowledge andskills,primarily innutritionsupport,across a diverse and vibrant area in central London.The postholder will be managing a varied caseload of people living in their own homes and care homes.Our team has adopted and is embedding a Food Based Approach to managing malnutrition and actively supports primary care with the appropriate use of prescribed oral nutritional supplements.The post holder will be involved in progressing the prescribing support strategy and this will include working with multi-disciplinary colleagues, training, education and resource development.

The post holder will bepart of a wider networkofother Nutrition Support Dietitians throughoutthe Trust.There is a strong emphasis on learning and development. We encourage all staff to participate in Quality Improvement and the Trust has an excellent Quality Improvement team who provide support and guidance in this process.

The position is full time, fixed term until 29/2/2024 and based at Lisson Grove Health Centre. We also welcome internal candidates looking for a secondment opportunity. This Trust is continuing to encourage agile working.

You will be responsible for:

  • Your own caseload of patients who require oral nutrition support
  • Actively partaking in quality improvement projects
  • Supporting student training

Job description

Job responsibilities

To have responsibility for providing a dietetic service (virtual/face-to-face) including appropriate dietary assessment and treatment in specific community settings as the service requires such as GP practices, health centres, a domiciliary service to clients in their own homes, nursing homes and residential homes.

To work autonomously to provide specialist dietary advice on a range of clinical conditions that will include a focus of a food based approach, oral nutrition supplementation and may include enteral feeding, to clients and carers. This will involve nutritional diagnosis and assessment (interpretation of biochemistry, anthropometrics, clinical condition and diet history) using evidence based guidelines and the development of treatment plans (calculation of nutritional requirements and client-centred goals) using evidence based outcomes.

To work autonomously to complete a nutritional assessment and recommend a food based approach to meet nutritional requirements or an appropriate prescription for oral nutrition supplements or enteral feed.

To use professional judgement and clinical expertise to interpret and analyse complex cases and information to formulate treatment plans according to evidence based practice and modify according to changes in clinical condition.
