Job summary

CLCH Community Planned Care Services are looking to expending our community services and currently seeking motivated dynamic band 5 nurses to join our District Nursing service based in Brent that will give you many opportunities to flourish and further develop your skills and career.

We are currently looking at a 5-year plan to upskill our nurses to become aspiring leaders for the future, this includes prescribing. This is an exciting time for our Brent District nursing teams, and we are looking at ways to include working together towards an integrated approach.

Main duties of the job

Are you looking for opportunities to develop your community nursing skills in your career and to be part of a unique and dynamic integrated care team? Do you want to be part of developing and shaping a new integrated care service in Brent?

Working within a structured 12 month work-based training programme to provide skilled nursing care to patients in a variety of community settings, working under the supervision of the District Nurse Team Lead (DNTL) and learning mentor to achieve clinical competencies that enable progression to a Senior Community Nurse B6 post at the end of the 12 month programme.

  • The post holder will undertake community nursing duties involving delivery of nursing services and holistic assessments of patients health needs using a person centred approach. This includes providing quality and seamless nursing care to housebound patients and being proactive in co-ordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long term care institutions.
  • In addition, the post holder will be involved in facilitating self care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery.
  • As a quality fellow the post holder will spend one half day per month working for their allocated shared governance council. Their role will be to support and encourage the councils work in delivering the quality strategy.
