The Barnet Integrated Discharge Team was formed as a response to Covid-19, and due to its success has secured funding until 31/03/2022. The Senior Clinical Co-ordinator role will give you an opportunity to support and develop a service, enabling timely discharges from acute settings to community providers.

Main duties of the job

You will be part of the leadership team for a small multi-disciplinary team of clinical and case co-ordinators, expected to work closely with the system-wide Health and Social Care Leads in expediting discharge times, minimising barriers and improving patient experience across all community discharges (Pathway 1-3) within the system.

You will have the opportunity to progress your leadership skills whilst growing and further developing this exciting service, along with working closely with, and gaining a better understanding of, all of the services providing health and social care to the local population.

  • To take a lead in the development and clinical effectiveness of the Integrated Discharge Team and support integrated system working in order to facilitate safe and timely discharge.
  • To ensure effective integration and support teams to deliver high quality and effective care and ensure optimal flow through acute, interim and final discharge destinations
  • To have excellent clinical knowledge, and use this to support complex discharges from acute and community settings, and draw on personal experience and skills/knowledge of all team members to improve discharge outcomes for patients and carers.
  • Have the responsibility to work as an integrated member of a system-wide Health and Social care multidisciplinary team to ensure the hub is meeting the patient and service needs, providing expert advice to ensure effective delivery of the service in accordance within the service specification.
  • To develop and promote robust multidisciplinary working across the wider health and social care sectors, that ensures integrated service and support networks for patients.
