Central London Community Healthcare Trust is a large community trust working in collaboration with Balham, Tooting and Furzedown PCN to recruit a dynamic and innovative senior clinical pharmacist.
If you enjoy supporting patients to get the best from their medicines, improving medication safety and quality of care, enhancing operational efficiencies or want to make an impact in a primary care setting then this could be the post for you.
The postholder must have post registration experience in hospital pharmacy or primary care and completed the CPPE Primary Care Pharmacy Education Pathway as well as independent prescribing qualification (or working towards both). A sound knowledge of prescribing and medicines management issues faced in primary care is essential with excellent decision-making and problem solving skills. Additionally, a wide exposure and well-rounded knowledge of various clinical conditions and their management. Compassion, flexibility, excellent comunication and time management skills are essential and the ability to build relationships with various health and social care professionals e.g. GPs, hospital clinicians, district nurses, carers, family, community pharmacists and care coordinators. Previous experience in primary care settings is desirable.
Main duties of the job
Key components of the post include:
Working as part of a PCN providing specialist expertise in the use of medicines, conducting structured medication reviews for patients with inappropriate polypharmacy and overprescribing, developing and implementing long term condition prescribing clinics and acting as a source of pharmaceutical expertise to various health and social care professionals e.g. GPs, hospital clinicians, district nurses, carers, families, community pharmacists and care coordinators.
The clinical pharmacist will be supported by our CLCH PCN pharmacy team having the opportunity to share good practice and develop the PCN service.
You will contribute to trust wide Medicines Management Service Provision including policy development, implementation of patient safety initiatives e.g. drug alerts, patient safety alerts, training for health professionals in the community across the trust and PCN.
The post will predominantly be working in GP practices within South West Merton PCN and will involve lone-working and travelling between sites. The postholder may also be required to travel to other sites and services within the organisation so will require flexibility as the organisation continues to grow. A car would be useful for travelling in outer London or outside of London but is not essential.

- To provide clinical pharmacist support to PCNs within Central London Community Healthcare NHS Trust (CLCH).
- To provide pharmaceutical support to health care professionals (HCPs) and patients within the PCN.
- To conduct face-to -face or remote structured medication reviews for patients in the clinic, practice setting, patients homes or within care homes.
- Support long term chronic disease management through polypharmacy review or specialist medicines optimisation prescribing clinics e.g. Diabetes/Asthma/COPD patients.
- Utilise clinical tools to proactively identify patients for structured medication reviews.
- Support achievement of national and local incentives and improving disease registers.
- To respond to patient medication queries regarding correct use of medications, side effects or other medication-related issues involving repeat prescriptions.
- Support cost effective, evidence-based prescribing in line with local and national guidelines.
- Support and improve prescribing and medication-related processes at a practice level.
- Work to reduce risk of medication incidents through review of local processes and systems around medicines management, supporting high-risk drug monitoring and improving prescribing safety.
- To build relationships with key stakeholders within the PCN, Integrated Care System (ICS) and CLCH
- Case manage patients who are frequently admitted to acute trusts as a result of medicines related problems.
- To ensure patients are supported to get the best out of their medicines in liaison with the persons GP, hospital clinicians, district nurses, carers, family, community pharmacists and care coordinators to ensure a multidisciplinary and integrated approach to medicines management, with a view to ensuring continuity of pharmaceutical care for patients in different healthcare settings.
- Identify high risk patients discharged from hospital and ensure appropriate follow-up in the community.
- To provide professional support and compose patient agreed medication care plans and communicate these with other health and social care professionals e.g. Community Pharmacists.
- Contribute to agreed meetings within and outside the organisation and represent pharmacy as appropriate.
- Respond to unpredictable work patterns and urgent requests for help by phone or email within a reasonable time frame.
- Plan and coordinate projects, quality improvement programmes or audits within the CLCH Pharmacy teams
- Collect appropriate quantifiable data to evaluate the service and identify areas for improvement and development
