Bank Community Dietitian - Merton

We are looking for an experienced Community Dietitian to join our team of Bank workers to work the Merton area.

The post holder will provide a clinical dietetic service to adults in Merton Community. This will mainly comprise of assessing and reviewing patients requiring home enteral feeding (HEF), oral nutrition support and dietary management for gastro conditions such as IBS in their own home and in care homes as well as regular virtual clinics.

Main duties of the job

Job Purpose

To provide a clinical dietetic service to adults in Merton Community. This will mainly comprise of assessing and reviewing patients requiring home enteral feeding (HEF), oral nutrition support and dietary management for gastro conditions such as IBS in their own home and in care homes as well as regular virtual clinics.

KEY WORKING RELATIONSHIPS

Secondary Care Dietetic colleagues, GP Practices, Care Homes, Community Nursing, Nutrition Companies, Home Feeding Company Nutrition Nurse, Community Pharmacists, AHPs, other primary care staff, patients and carers in the Merton area.

MAIN DUTIES AND RESPONSIBILITIES

1 CLINICAL RESPONSIBILITIES

  • To work autonomously to provide HEF and oral nutrition support advice and therapeutic support to patients, carers and other healthcare professionals via domiciliary and care home visits. This will require collation of complex information including nutritional assessment, diagnosis and interpretation of biochemistry, anthropometry, clinical condition, and diet history.

  • Development of treatment plans which include calculation of nutritional requirements, client centred goals and treatment outcomes.

  • Use of a food-based approach to achieve specific patient centred goals.

  • Provide evidence based dietary treatment for gastro conditions following national guidelines, taking into consideration socio-economic and lifestyle factors.

  • Prescription of dietary treatment using ACBS products for conditions adhering to professional / clinical guidelines and supporting the organisations agenda for cost and clinically effective ONS prescribing.

  • To provide practical and safe tube feeding plans to patients, community nurses and/or carers for use in the community.

  • To advice on and order nutrition and ancillary products to support safe and effective home enteral feeding.

  • To assist in the management of the dietetic caseload ensuring high quality dietetic care is provided. This will include the review of patients at appropriate intervals according to prioritisation criteria and clinical judgement.

  • Line management responsibility of Dietetic assistant and provide clinical supervision to junior members of staff.

  • To work in a co-ordinated multidisciplinary manner, communicating effectively with other team members and the wider clinical network to promote timely interventions including Home Feeding Company Nutrition Nurse, Speech and Language Therapists, and Community Nursing Team.

  • To use professional judgement and clinical expertise to independently interpret and analyse complex situations and information to formulate treatment plans according to evidence based practice and modify according to changes in clinical condition.

  • To be clinically competent to give nutrition support advice, treatment and dietary counselling to support behaviour change and adherence to long term treatments.

  • To communicate complex and sensitive information effectively with patients from a wide range of backgrounds, carers and families, tailored to their needs, to gain consent, ensure understanding of condition, treatment options, risks, acceptance and compliance with dietary treatment, which may include unwelcome or difficult to accept information e.g. life time adherence to a artificial diet or limitations of treatment in terminal illness.

  • To apply developed communication, negotiation and counselling skills to empower patients to achieve long term lifestyle and behavioural changes taking into account emotional, physical, social and psychological barriers as well as strong influences on change e.g. depression, dementia, language difficulties, resistant attitudes, aggression, behavioural problems, and mental illness. To effectively use interpreters, sign language, information technology or other means of communication appropriate to the clients requirements.

  • To discuss ethical issues with service users and health professionals.

  • To participate in multi-disciplinary / multi-agency collaborative workings to promote continuity of care.

  • To use clinical judgement to refer clients to other services and professionals when appropriate.

  • To be ultimately responsible for managing a clinical caseload.

  • To be professionally and legally accountable for all aspects of own work and ensure that own work is within professional standards and clinical guidelines.
