We are looking to recruit a Band 6 / 7 DSN to join our dynamic team for fixed term maternity cover between now and Jan 2023.

We are a multidisciplinary team consisting of Consultants, Dietitians, Podiatrist as well as Specialist Nurses. The team receives referrals from many primary care and community services as well as local acute hospitals.

We are a unique community provider as we manage people with both Type 1 and Type 2 diabetes and are an initiating centre for Freestyle Libre. We also deliver DAFNE & DESMOND structured education.

We are based in Morden (at the end of the Northern line) and work within the London Borough of Merton. Morden Underground station is just around the corner and Wimbledon Station 1.5 miles away. We are also on many bus routes, with a tram station very nearby.

Main duties of the job

So, what do you need to join our team?

  • Relevant post registration training and experience in the management of patients with Type 1 & Type 2 Diabetes
  • Ideally be an Independent Nurse Prescriber or demonstrate a willingness to achieve this qualification
  • Be a great a team player who is willing to work collaboratively with the multi-disciplinary team to deliver the best care possible for our patients
  • Have a strong commitment to the delivery of high quality, safe, patient care and be a good role model in reflecting the Trusts core valuescompetent to offer guidance and supervision to members of the clinical team

To work closely with the Service Lead to provide a Tier 3 Diabetes Specialist Nursing Service, that contributes to the strategic planning and delivery of the multidisciplinary Community Diabetes Service and is in line with the Trust objectives.

To be a key member of the multidisciplinary Community Diabetes Service to plan and deliver high quality integrated diabetes care in the community across Tiers 1 3.

To provide specialist assessment and treatment of patients with diabetes, liaise with other agencies and health professionals and maintain accurate clinical records.

  • Degree in Nursing or equivalent
  • Current registration with the NMC, Showing a registered General Nurse.
  • Relevant post registration training in Diabetes e.g.: Masters or diploma in diabetes management
  • Independent Nurse Prescriber or be able to demonstrate an ability that will enable attendance of a prescribing course within 6 months of starting post

Desirable

  • Teacher/Training qualification
  • Accredited trainer in DESMOND/DAFNE
  • Supervisory skills training
  • Post registration training in Cognitive Behavioural Therapy

PHYSICAL

Essential

  • Able to carry out the duties of the post with or without adaptations
  • Able to carry out moderate physical effort throughout the day

EXPERIENCE

  • Extensive experience working in Diabetes either in primary or secondary care
  • Highly Experienced in managing a clinical case load
