Job summary

Due to a staffing review we have created 8 Band 6 senior rehabilitation nurse roles for our 33 bedded rehab unit. These roles are unique as they rotate into the unit's admission screener post to develop your operational skills and working with our acute partners.

We work as multidisciplinary team to ensure we deliver high-quality person-centred care in an environment that is friendly, patient and staff focused and rewarding. You will be working with a varied multidisciplinary team consisting of; Gps, Nurses, Occupational therapist, Physiotherapist, Speech and Language Therapist, Dietitian, rehabilitation assistants and podiatrist.

Our bedded rehabilitation service aims to empower patients to recover from episodes of ill-health and optimise their level of independence enable them to resume living at their usual residence.

This is a 24 hour a day 7 days a week service operating 365 days a year therefore the post holder will be expected to work shift patterns to cover the service including Early, Lates, Long Days, Nights, or a combination of both.

Main duties of the job

  • To be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care
  • To work in a collaborative manner and liaise with other professionals and agencies to ensure a high-quality service for patients
  • To lead and monitor the performance and work of the nursing team
  • To provide a stimulating environment for patients and carers
  • To be part of the Clinical screening rotation
  • To ensure the effective running of the Ward in the absence of the Ward Manager

Job description

Job responsibilities

  • To provide skilled, effective and evidence-based nursing care to patients in the Rehabilitation ward
  • To identify, prioritise and implement programmes of care that meet the needs of the patients
  • To promote the participation of residents and relatives in assessments and care planning
  • To ensure that all care plans in their unit are fully completed, up-to-date, and evaluated regularly
  • To ensure each Patient has a comprehensive risk assessment and management plan documented in the care plan
  • To support junior members of staff to complete, implement and evaluate care plans
  • To participate in, and help others to participate in, regular multi-disciplinary clinical reviews
  • To liaise with, and where appropriate, initiate referrals to, all those involved in the delivery of care to ensure adequate care and support to residents and carers
  • Make sure that patients are treated with respect by ensuring their privacy and dignity are protected and by making sure they are addressed in the way they choose
  • Resolve problems for patients and their relatives by acting quickly to deal with problems when and where they occur and by working closely with the Patient Advice and Liaison Service and Complaints Team
  • Resolve problems for Staff by acting quickly to deal with problems when and where they occur and report back to the Ward Manager with actions undertaken
  • To actively promote resolution of verbal and written concerns/complaints at local level, undertake investigations to formal complaints & formulate responses & implement any actions required
  • To ensure the safe handling of body fluids and contaminated sharps
  • To provide specialist nursing skills and knowledge such as continence assessment and management, falls assessment and prevention, pressure sore prevention and management, medicines management
  • To undertake MUST nutritional assessments and advise and support residents with their nutritional needs e.g. PEG feeds
  • To negotiate with patients to engage with their care in order to avoid complications such as falls, pressure sores, infections and to optimise their health and wellbeing
  • To be alert to the needs of adults at risk, identifying risk of possible harm and taking appropriate safeguarding action as required
  • To have an innovative approach to practice in response to changing service needs and priorities
  • To have knowledge of, and be able to effectively use, local services and resources to promote Patient care

  • RGN / RN with current registration
  • Post -registration qualification in older people's nursing or equivalent
