Job summary

CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

An exciting opportunity has arisen for a Healthcare Assistant to join our Community Adult Planned Care team based in Harpenden Memorial Hospital providing care in Care Homes across West Herts. The team consists of Community Nurses, District Nurses and Health Care Assistants. The Community Nursing team covers 8am-8pm, 7 days a week. Our aim is to be the best we can, offering safe, patient-centred and evidence based care to those accessing our service.

Main duties of the job

The successful candidate will be enrolled into our 12 week induction programme. You will also have regular input from the Professional Development Nurses and your Team Leader to support you in gaining your competencies. As a service we offer a range of different opportunities to develop your skills.

We offer flexible shifts and the successful candidate will be required to work a minimum of 1 in 4 weekend.

  • The Health Care Assistant will provide a mixture of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional. The HCA will work with patients in care homes across Hertfordshire.
  • The post holder will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The HCA will promote optimum independence and individualised client centred care to a delegated caseload
  • The post holder will also support the unplanned care team to deliver against key performance indicators and maintain quality of the service. The service is provided seven days per week 8pm-8am covering the four localities across Herts Valleys.
